Download Emergency Contact Form

It is important that the school has full information on each dance student, in case of emergency.  Not everyone has Microsoft Word, so the form is also provided as an image, which can be saved and/or printed on your computer as if it was a picture and then filled in by hand.

The emergency contact form can be downloaded as an image HERE (print it and fill it in by hand) or as a Microsoft Word document HERE (fill in on screen, or print and complete by hand).  Please fill in a form for each child you have attending the dance school and pass the completed form(s) to Miss Kate.  Free Microsoft Word Reader download HERE

If the dancer's contact or medical details change, please fill in a fresh form.  Thank you.

Notes

DANCE CLASS NOTES

All Classes are multidisciplinary, in that each child will be taught elements of ballet, tap and modern/jazz dancing.

Newcomers should wear comfortable clothes in which they can stretch, bend and leap etc.  Once the decision has been made to continue classes then the appropriate dancewear must be worn.  Supplies are available at very reasonable costs from the school and prices are posted on the noticeboard in the foyer.  (Pre-schoolers are welcome to wear their favourite leotard/tutu)

Pre-school children must be accompanied throughout the class by a responsible adult.  Parents/carers may watch the older dancers' classes on the final session of each term.

Basic dancewear is a royal blue leotard for the juniors and burgundy for the senior dancers; ballet shoes and (for school age dancers) tap shoes.  Wootton Bassett School of Dance T-shirts are also available, and with black lycra jazz dance trousers are the favoured dancewear of our more body-concious older dancers!  Jazz/modern dance shoes are required for some public performance shows and festival dances, and are also permitted in class.  Optional Wootton Bassett School of Dance fleeces (ideal for walking to and from classes) in black with a pink logo, are also available. All essential dancewear is obtainable from Miss Kate.  Prices are posted on the school's notice board or see Miss Kate.  If dancers wish to upgrade their tap shoes, heel taps are available from Miss Kate.  The trophy/key-cutting/cobbler in Apsley Arcade will fit heel taps, but does not stock his own supply.

LEOTARD WASHING INSTRUCTIONS

All leotards should be hand washed inside out, preferably in cold water and immediately hung to dry on a coathanger. Keep away from heat.
DO NOT: Use bleach or detergents.
DO NOT: Tumble dry or iron.
DO NOT: Rub or scrub foil print or metallic garments.
DO NOT: Hang over radiators.
Foil print and metallic finishes may fade or dull with wear and repeated washing. Areas that incur friction or abrasion may show some loss of foil.
