This is not an adoption form.  This is basically a very generic intake form.  It will give us an idea of what kind of reptile you are interested in adopting and what, if any, experience you have with the reptile you want to adopt.  Once we receive this form, we will contact you via telephone for an interview prior to making a decision as to whether or not to approve your request.  If your request is approved, you will be sent an adoption contract to sign and return, along with the adoption fee.  NOTE:  I will not ship iguanas nor large boids.  You must pick those reptiles up in person from the shelter.
