My current professional fee schedule is $165 for an initial 50-minute consultation. Subsequent sessions (50 minutes) are $140 per visit. Hypnosis for smoking cessation is a one-time fee of $80 for a half-hour, individualized session. Missed session or late cancellation fee is $20. There is also a $30 returned check fee for uncleared checks.
Since insurance may require a pre-authorization; and since insurance reimbursements vary, please contact you insurance carrier for related co-pay and / or deductible payments (if applicable), prior to your first appointment. If your insurance is not accepted, please be advised that many policies also allow for "out-of-network" provider benefits.
