Cialis (Tadalafil) is a second generation selective phosphodiesterase
5 (PDE-5) inhibitor medication - Viagra (Sildenafil) being the first
generation. PDE-5 inhibitors like Tadalafil work by blocking an enzyme
called phosphodiesterase-5, or PDE-5. This helps the smooth muscles in
the penis relax and widen, which allows for more blood to enter, thus
increasing the body's ability to achieve and maintain an erection
during sexual stimulation. Tadalafil will not work to cause an
erection without physical stimulation to the penis, such as that
occurring during sexual intercourse.

Precautions

You should undergo a complete and thorough medical and sexual history
and physical exam to determine the cause of your impotence before
taking Cialis. Men who have medical conditions that may cause a
sustained erection (Priapism) such as sickle cell anemia, leukemia, or
multiple myeloma or who have anatomically an abnormally shaped penis
may not be able to take Cialis. The cardiac risk associated with
sexual activity need to be discussed prior to initiating any treatment
for impotence. There are several medications that are know to interact
with Cialis via the hepatic cytochrome P450 metabolism and might
either increase the plasma concentration of Cialis (Cimetidine,
Erythromycin, Ketoconazole, Itraconazole, Mibefradil) or decrease the
available drug concentration (Rifampin). It has been also reported
that loop and potassium sparing diuretics might increase the available
systemic concentration of the drug. Make sure to tell your doctor
about all medications you are taking including those you can get
without prescriptions. In elderly patients, in patient with severe
kidney or liver disease the drug concentration might be increased and
caution in drug dosing is indicated. Cialis has not been studied with
other treatments for impotence, so use in combination with other
treatments is not recommended

Side Effects

Cialis should not be used by patients taking nitrates (such as
nitroglycerin tablets or patches) or any alpha blocker other than
FLOMAX 0.4mg daily because the combination of these with Cialis could
significantly lower blood pressure and lead to fainting or even death
in some men.
