Adoption Application

1. Chinchilla Adoption Preference *Please give details of what colour, gender, age of the chin you would like to adopt. Please also advise if you would be willing to adopt a 'special' chin - one that may not look so perfect (eg, amputation).
2. Personal Information *Please provide your name, email, address & contact phone/fax numbers. Please also advise if you are over 18 years old.
3. About Your Home *Please advise how long you have been living at your current address. If you are renting, do you have your landlords permission to keep chinchillas and are we allowed to contact your landlord (if so please provide details)?

I am committed to having a chinchilla for the 20+ yrs they can live for.
I can provide a safe room away from other animals for the chinchilla to have daily exercise outside of its cage.
I can and will provide adequate veterinary treatment if the chinchilla becomes sick or injured.
I will return the chinchilla to the Rescue Centre if for whatever reason I cannot keep it.

5. Your pet ownership history

1. Do you currently have any other animals including chinchillas? Please provide all details, age, sex, breeds & housing (chin), what you feed (chin), and if you will be willing to show Vet Records.

2. How many pets have you owned in the last five years? If any are no longer with you, please tell me why.

3. Have you ever owned a Chinchilla before? Please give all details of past ownership, age, sex, food type, if passed away give details of why and if you will be willing to show a Vet certificate.

4. Are all your other pets spayed or neutured? If they are not, please tell me why.

5. Have you ever surrendered a pet to a shelter, given up a pet for Adoption, sold a pet to a Pet Shop, Auctioned or Sold in any other way or had a pet put to sleep (this is including if they were for breeding)? Pease provide all details.

6. May I contact your vet and/or other vets you have seen? Please give details.

7. If you do have other animals, do you flea, worm, get regular routine vet checks done? Please give details of how you care for each one.
