Students
Any student, Male or Female between the ages of 13 and 18 inclusive may apply.

Send the completed forms with a cheque for £20 (Deposit) payable 'Lincoln Harmony Camp' to:

info@harmonycamp.co.uk including your name and address and listing which forms you require, and they will be sent to you.

The balance of £130 will be required by 1st July 2009

STUDENT CAMPER
CODE OF CONDUCT

Harmony Lincs (Lincoln BHC) are proud to host 'Harmony Camp 2009' and we are very pleased to have you attend. In order to ensure that your experience is safe and enjoyable, we require all student campers to abide by the following Code Of Conduct:

1. All student campers are required to refrain from smoking at any time during camp. The Bishop Grosseteste University College is a non-smoking campus.

2. The use of bad language will not be tolerated at any time during camp.

3. The use of non-prescription medication or prohibited drugs is strictly prohibited.

4. The consumption of alcohol on campus is strictly prohibited.

5. There are to be no male campers on the sleeping floors of the female students accommodation at any time for any reason.

6. There are to be no female campers on the sleeping floors of the male students accommodation at any time for any reason.

7. All students are requested to be at the scheduled place at the scheduled time. No one will leave their appointed group without a camp appointed chaperone.

8. Students are not authorised to go off campus without the permission of the Camp Director and must sign out at the camp office.

9. Students are to be in their accommodation rooms at the designated times. The only exception being a medical emergency.

10. Students are expected to follow the instructions of the camp faculty and mentors, and the campus rules as issued by the Bishop Grosseteste University College.

11. Mobile phones must be SWITCHED OFF during rehearsal sessions.

PARENT / GUARDIAN
RELEASE FORM CONDITIONS

THIS ONLY APPLIES IF THE STUDENT IS UNDER 16

In consideration of the teaching, coaching and musical experience being offered by Harmony Lincs, I hereby release and discharge Lincoln Barbershop Harmony Club, both as an organisation and its individual members, individual members of the Harmony Camp faculty, staff and volunteers, the British Association of Barbershop Singers (BABS) and the Bishop Grosseteste University College from responsibility and liability for injury to, or the loss or damage to property, arising during or in relation to Harmony Camp 2009, unless directly and proximately caused by the negligence or fault of said individuals or groups.

I agree to hold harmless the Lincoln Barbershop Harmony Club, individual members of the Harmony Camp faculty, staff and volunteers, the British Association of Barbershop Singers (BABS) and the Bishop Grosseteste University College for any action brought against said individuals or groups, arising out of Harmony Camp 2009, except to the extent such action establishes the negligence or fault of said individuals or groups.

Data Protection Act 1988 - As it is our intention to maintain a photographic and video record of Harmony Camp 2009, along with a written record of each student's progress, it is a requirement of the Date Protection Act 1988 for permission to be sought for this.

By singing the Parent / Guardian release section of the Student Camper Application Form, I agree all the above conditions.
