Furthermore, I acknowledge that I have been notified and fully understand that I must submit the registration form and full induction fee to the World Martial Arts Hall of Fame, along with a current Certificate of Rank, a one page Martial Arts resume (typed and photo ready), two (2) passport size photos of myself and one (1) 8x10 black and white photo (action or still) of myself no later than May 1, 2007 (no exceptions). I understand that these items will be used by and become the sole property of the World Martial Arts Hall of Fame.

Furthermore, I release the World Martial Arts Hall of Fame, Dr. Miguel E. A. Moore, and any and all employees, members, instructors, relatives, news media, or anyone else connected or associated with The World Martial Arts Hall of Fame and the World Sports Medicine Hall of Fame, from any liability associated with the use of any or all personal information provided by me pertainng to my Martial Arts history and/or background.
