I, on behalf of myself, my heirs, my assigns, personal representatives, and next of kin, agree to indemnify and hold harmless Greater Orlando Homeschool Athletics, its agents,volunteers, coaches,representatives, sponsors, families, as well as any other person or entity connected with this program, and their heirs, from all liability, claims, demands, damages, or costs for or asiring from this program, as scheduled and coordinated by volunteer and paid representatives. I am aware that, as with participation in any physical activity, there is a risk of injury , and while particular rules, equipment, and personal discipline reduces this risk, the risk does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of other participants. I am aware if I observe such risks I am responsible for removing my child from the situation I feel is hazardous. I release Greater Orlando Homeschool Athletics from all liability and indemnify Greater Orlando Homeschool Athletics against any and all claims and judgements arising from this participant's/my child's association with Greater Orlando Homeschool Athletics. I am aware my presence, or the presence of a guardian which should and will be made known to both the program coordinator as well as the head coach/clinic instructor PRIOR TO BEGINNING THE PROGRAM, is required at all times that my child is participating in the program or its practice sessions.
I know my child needs a mouthguard to be worn at all times in order to participate in flag football.
