Dosimeter Calibration
Dosimeters must be recalibrated by Jan. 1, 2010. REMSCO has made arrangements for this to be done by the manufacturer at no cost to the squads. Get the RMA form here. Contact REMSCO for help in completing the form.
H1N1 Vaccination for EMS Volunteers
NYC Dept. of Health will be offering free H1N1 vaccinations to all ACTIVE AMBULANCE personnel from the local volunteer ambulance companies. Dates and locations to be announced soon!
Reportable Incidents
Title 10 New York State Codes, Rules and Regulations - Part 800.21(q) require the EMS service to report certain types of incidents to the Department within 24 hours of the event and in writing within five (5) business days. Get the form here or visit the DOH website for additional information.
Certified EMS Agency Information Update Form
When there is a change to an agency Chief Officer (due to elections, promotions, etc.), agency address or contact information the NYS DOH should be notified by filing form DOH 2936.
