Prior to running an event, the online event registration form (link below) must be completed and submitted. The event planner form (link below) must be filled out and given to the person of authority at the venue where the event is to take place no later than 2 weeks prior to the event.
Online event registration form
After the event has concluded, the online event report from (link below) must be completed and submitted in order for the Survivor to receive credit for the event. If the Survivor chooses, he or she may fill out and keep an event report for their records (link below). Keeping a hard copy of event reports is not required but highly recommended.
