Blue Chip Application-New or Continuing University of Arizona Student
Any undergraduate student of any year in school can apply to join the Blue Chip. Applications are due July 1 each year for a start date of the Fall semester. There is no application fee.
Blue Chip Application-Pima Leadership Institute and PEAK Leadership Institute Transfer
Pima Community College students who have completed Levels 1 and 2 of the Pima Leadership Institute and Paradise Valley Community College students who have completed the Gold or Platinum levels of the PEAK Leadership Institute are eligible for entry to Blue Chip directly into Phase II. Applications are due by April 15 each year for a start date of the Fall semester. There is no application fee.
Blue Chip RSVP Form
All incoming Phase I students need to fill out and submit the RSVP form in order to participate in Blue Chip. This form needs to be submitted no later than May 1, 2008 to ensure participation in the program.
Blue Chip Scholarship Form
Blue Chip would like to ensure access to the program by offering several financial need scholarships for program fees. If you would like to apply for a scholarship, you need to submit your form prior to the first day of classes of each semester.
Blue Chip Exit Form
In order to officially leave or not continue in the Blue Chip Program, you will need to fill out the Blue Chip Exit Form prior to the first day of classes to ensure not being billed for the semester program fee.
Blue Chip Temporary Leave of Absence Form (Study Abroad, Internships, etc.)
If you are planning to take a leave of absence from Blue Chip for a semester, year, etc. to study abroad, pursue and internship, etc. but would like to be active in the Blue Chip Program upon return, fill out the Blue Chip Temporary Leave of Absence Form. You will need to fill out the Blue Chip Temporary Leave of Absence Form prior to the first day of classes to ensure not being billed for the semester program fee.
Blue Chip Core Value Award Form
Nominate yourself or another Blue Chip student who you believe lives one of the Blue Chip core values of Integrity, Diversity, Service, or Excellence. Four awards for each core value will be awarded (one for each phase). Application deadline has passed.
University of Wollongong Black Opal Exchange Application
Interested in studying abroad next Fall at the University of Wollongong and participating our partner program, Black Opal? Fill out this application to be considered for selection. Up to 5 students will be selected to go on exchange each year. This supplemental applications is due April 1, 2008. All interested candidates must also fill out the Study Abroad Application at http://www.studyabroad.arizona.edu/.
Theme Facilitator Application
If you are interested in being a Theme Facilitator for a Phase II theme class in the Fall semester, you can apply online. Theme Facilitators serve as Teaching Assistants in the Fall semester for the theme class and advisors to Phase II case study teams during the Spring semester. Application deadline has passed.
Assistant Theme Coordinator Application
Each theme has an Assistant Theme Coordinator (ATC) who advises Phase III students in their assigned theme. Each ATC also serves on the Blue Chip staff planning Blue Chip events, doing office hours in the Blue Chip area, and assisting in decision-making for the program. Application deadline has passed.
Blue Chip Involvement Application
Get involved with Blue chip in a variety of positions by filling out this application. Application deadline has passed.
Programming Team: Plan and advertise Blue Chip events such as Family Weekend barbecue, Homecoming Pancake Breakfast, Closing Events, Semester Socials, and more! Find exciting and creative ways to bring Blue Chip students together while enhancing your event planning skills. Fall and/or Spring semester
Service Coordinator: Create partnerships with community organizations and plan service opportunities for Blue Chip students to participate in. Also assist in planning and facilitating the Fall and Spring Into Service Days with the University Activities Board. Fall and Spring semesters
Recruitment Team: Help to spread the word and generate interest in Blue Chip for students and parents. Share your story of Blue Chip with students at Orientation sessions throughout the summer and at recruitment opportunities throughout the year. Fall, Spring, and/or Summer
Alumni Relations Team: Promote Phase V, the alumni phase of Blue Chip. Assist in contacting alumni and sending out information about Blue Chip and our current programs. Assist in planning alumni events including partnering with the Programming Team for the Homecoming events. Fall and/or Spring semester
Fireside Chat Coordinator: Develop Fireside Chat program for Phase III students. Contact both
Historian: Take pictures, create a scrapbook, document Blue Chip’s activities. Be a part of creating Blue Chip history! Fall and/or Spring semester
Marketing Team: Assist in creating up-to-date and exciting marketing materials for Blue Chip. Work with the Leadership Programs and Union Marketing staff to design the Blue Chip t-shirt and planner. Fall and/or Spring semester
Newsletter Team: Each semester, a leadership newsletter will be sent out to Blue Chip students, parents, and students and advisors on campus and across the country. In each newsletter, there will be a section highlighting Blue Chip happenings and accomplishments. Collect articles, document events, and write sections of the leadership newsletter.
