General Admission Requirements for FRESHMEN:

Any graduate from public or DepEd recognized private elementary schools and PEPT passers may apply for admission provided that he/she present the following at the Registrar's Office upon registration:

• Elementary Progress Report Card/DepEd Form 138-A (Original Copy)
• Elementary Pupil's Permanent Record/DepEd Form 137-A (Original Copy)
• Certificate of Good Moral Character (Original Copy)
• Certificate of Live Birth (Certified True Copy)
• 4 copies 1x1 ID Picture
• Certification that the applicant is a working student (for Night Classes)

General Admission Requirements for TRANSFEREES:

Any high school student from public or DepEd recognized private school and PEPT passers may apply for admission provided that he/she present the following at the Registrar's Office  upon registration:  

• Secondary Progress Report Card/DepEd Form 138-A (Original Copy)
• Secondary Student's Permanent Record/DepEd Form 137-A (Original Copy)
• Certificate of Good Moral Character (Originbal Copy)
• Certificate of Live Birth (Certified True Copy)
• 3 copies 1x1 ID Picture
• Certification that the applicant is a working student (for Night Classes)
