This puzzle box was screen-used in Hellraiser: Deader.  One of several used on screen.  COA from Premiereprops, and also signed by a representative from Miramax Films.  The box was purposely aged and distressed for filming.  It's made from solid wood with brass etchings, and has a nice weight to it.
